Possible since 2006 (court-granted)
Korean Supreme Court allows change of legal gender. A Feb 2020 revision of the Court's administrative guidelines demoted sterilising/genital surgery from an item courts must investigate to a mere reference item, so some courts now grant a change on consistent identity + hormone use with no surgery — but others still treat surgery as a bar, and in May 2023 the National Human Rights Commission recommended the guidelines be revised again for exactly that reason. Practice remains inconsistent by court. Foreigners with F-visa long-term residency can pursue this with a Korean lawyer.
Legal, prescription-required
Hormones are Rx-only in Korea but routinely prescribed by endocrinologists for trans patients. Carry your prescription bottle when traveling — customs may ask.

No comprehensive anti-discrimination law
A national LGBTQ anti-discrimination bill has been stalled in the Assembly since 2007. Seoul and some other cities have local human-rights ordinances that include SOGI, but enforcement is weak. The 2024 Supreme Court ruling extended limited spousal-benefit rights to same-sex partners — slow progress but real.
Trans women — case-by-case exemption
Trans women assigned male at birth are evaluated by the military medical board; exemption is the standard outcome but requires documentation. Trans men assigned female at birth are not conscripted (Korea doesn't conscript women).
Possible with home-country documents
You can update your Alien Registration Card name + gender after your home-country passport reflects the change — bring updated passport + court documents to immigration.
